Office Policies

Cancellation Policy
Please help us serve your family better by keeping scheduled appointments. If you need to cancel or reschedule an appointment, it must be done at least 24 hours in advance.

No shows and same-day cancellations for check-ups, consultations, or chronic problem follow-up (ADHD, Asthma, Weight Management, etc.) will result in a $25.00 charge per child.

Multiple missed appointments may result in dismissal from the practice. If you are more than 15 minutes late for your appointment, you will likely need to reschedule.

Treatment of a Minor  
All children under the age of 18 must be accompanied by a parent or legal guardian. If a patient comes with another adult (18 years or older) designated by the parent, the Authorization to Treat a Minor form needs to be filled out with the name of the authorized party, signed by a parent, and on file with our office. Children 16 and older may come unaccompanied for visits only with a signed authorization on file from the responsible party.

Adult Patients (18 years and older)
Once your child turns 18, they are legally considered an adult and control their medical information under HIPAA. Without the 18-year-old’s consent, healthcare providers cannot release their medical records to parents or others. Providers also cannot discuss a patient’s health status or treatment without their consent, even if the young adult is on their parents’ insurance plan. Any 18-year-old must complete a consent form for us to release information to a parent or guardian.

Double Well Visit Policy
At our practice, we want to ensure each child receives the time, attention, and quality care they deserve during their wellness visits. For this reason, we cannot schedule more than two children for well visits at the same time. If your family needs appointments for more than two children, we are happy to assist you in scheduling additional visit times so that each child’s developmental, physical, and emotional needs can be thoroughly addressed.

Separation and Divorce Policy
Maryland Farms Pediatrics will remain impartial in all divorce and separation matters. Our primary goal is to provide high-quality pediatric care for our patients. We assume children are under joint custody of their parents, with both parents having the right to access their child’s medical records and make medical decisions unless provided with court documents stating otherwise. The parent present at a visit will be presumed to have medical decision-making rights.  Parents are expected to establish communication systems independently without involving our staff as intermediaries, specifically regarding scheduled appointments, visit diagnoses and treatment plans, and payment arrangements.
